We also accept select HMO plans from the San Francisco Bay Area where the medical home is Brown and Toland or Hill Physicians.

Contact your insurance provider
To confirm your plan's coverage, we recommend reaching out to your insurance provider. They can provide you with detailed information about your coverage, including any co-payments or out-of-pocket costs that you may be responsible for.
Provide our NPI and Tax ID
If asked, reference our NPI number, which is 1386855120, and our Tax ID, which is 474883537, for an eligibility check. This will help your insurance provider confirm that we are in-network and covered under your plan.
Consider making one of our providers your PCP
Some insurance providers may require you to select one of our providers as your primary care provider (PCP) to ensure that your charges are in-network. If this is the case, you can provide our chief medical officer, Dr. Nicole Tsang, DO, as your PCP.
Scan your insurance card using the mobile app and we will tell you if you’re in-network, the amount of office visit copays, and the status of your deductible.
If you have an HMO plan, call the number on your health insurance card or go to your plan’s website before the appointment to select a Circle Medical physician as your PCP.
